Patients can refer themselves into the service by completing a Community Podiatry Service Application Form which can be completed by someone on the patient's behalf such as a GP, health professional or family member.

If you are completing an application form for a home visit request, please read the Podiatry Home Visiting Leaflet. If a home visit is requested, both a Community Podiatry Service Application Form and a Podiatry Service Mobility Questionnaire must be completed.

The service has a maximum waiting list of 18 weeks.

If you are unable to make an appointment please phone us as soon as possible, so the appointment can be made available for another person. Please contact the service using the contact telephone number on the appointment letter. 

Your Appointment

Appointment times vary between 20 and 40 minutes and take place during normal office hours.

You will need to bring your regular footwear, your current medication list and any insoles or devices you have been prescribed for your feet. 

Wearing loose fitting trousers is beneficial for some assessments.
