Anyone with a foot problem can refer themselves into the podiatry service by completing our referral form.
To do this, please click on to the Community Podiatry Service referral form. This link will download a word document to your phone, device (tablet or i-pad) or computer.
For people unable to attend a clinic, we have a limited service provided in the home. Please read the Podiatry Home Visiting Service information for more information. If a home visit is requested, please complete the referral form Community Podiatry Service referral form and answer the questions about home visits.
Please complete the form or ask someone to help, such as a family member, carer or health professional. To help our clinical team we would also appreciate a photo of your foot problem (if you are able). Once you have completed the form, please email the form and the photo to the podiatry department (email address is on the form).

Please note: We aim to have a maximum waiting time of 18 weeks. To help those most in need, the service will prioritise urgent problems like wounds, infection and disabling pain.
If you are unable to attend
Please phone us as soon as possible, so the appointment can be given to another person.
Please contact the podiatry service by using the contact telephone number on the appointment letter for the local health centre you are attending.
Appointment times vary between 20 and 40 minutes and take place during normal office hours.
You will need to bring your regular footwear, your current medication list and any insoles or devices you have been prescribed for your feet.
Wearing loose fitting trousers is beneficial for some assessments.
