We will assess your health case management needs and work with you and your relevant other to ensure the services you need are individual to you. We will co-ordinate your care and review services to ensure you are getting the right support. We work across hospitals, community beds and in your own home.

We offer tailored support for people with complex health and social care needs, enabling them to live independently at home for as long as possible. This helps them stay as well as possible without the need for urgent hospital visits, come home after being in hospital to recover and avoid admission to a care home.

We encourage patients to have choice and control over their care, so they can self-manage their health, support them with continuing healthcare funding and provide end of life care when fast track funding has been authorised.

Who's it for?

Those over the age of 18, and have a Leeds GP who are eligible for NHS Fast Track funding and continuing healthcare.


Referrals may be made by any health or social care professional involved in a patient’s care in the community such as:

  • GP
  • Community Matron
  • Community Nurse
  • Hospital professional (in relation to individuals registered with a Leeds GP)
  • Therapist
  • Social Worker
  • Leeds Continuing Care colleague
