How can you refer to the service?

We have created a single point of access for ICAN services, to make the referral process safer, more efficient and easier to use.

Most people are referred to the service by their GPs. Some people will be referred by Leeds Teaching Hospitals NHS Trust, or by their health visitor or school nurse.

Please use the specialist referral forms for:

General referrals 

For general ICAN referrals and referral criteria please use the form below (this includes Community Paediatric Clinics, Paediatric Neuro-disability, Audiological Medicine, Growth and Nutrition, Occupational Therapy and Physiotherapy):



Inclusion Nursing 

In response, families and referrers will be notified about what will happen next and where appropriate will be offered an appointment. If the information on the referral form is insufficient, referrers will be advised and the referral will be rejected until additional information is provided. The referral may be forwarded on to a more appropriate service or returned with advice on alternative services.

Springfield Neonatal 

Referrals are from Forward Leeds, and Leeds Teaching Hospitals NHS Trust Midwifery Services or occasionally direct to the service on 0113 843 2000.

Adoption and Fostering 

All referrals come directly from the allocated social work team.
