What does your average day look like?
We plan our own diaries so when we come in in the morning we look at emails and outstanding work from the day before. If we’re on duty, we also go through duty referrals. Sometimes, there are discussions and meetings with other health colleagues and partners. We may also have to attend Multi-Disciplinary Team meetings. Early afternoon, some of the different clusters come in and have their meetings and if any of our cases are discussed in these we join in. At the same time there might be an emergency - we obviously put these first. We also prepare for patient visits - who we’re seeing, what it’s for and what information do we need to discuss so we can plan care packages.
What’s the best part of your role?
Being able to make a difference to people’s lives. You get a referral where someone is not managing well but you can go out, meet them and build a relationship. By listening we can determine if we’re able to provide a service which meets their needs.
What do you find most challenging?
When there’s crisis situations - we have to work with other colleagues to solve these. Also when a number of different cases need attention at the same time.
