Referrals Process and Criteria

Inclusion Criteria:

  • Patients aged 60 years and over who agree to the assessment.
  • Patients who are registered with a Leeds GP.
  • Positive answer to 3 or more of Falls Risk Assessment Tool (FRAT) – see referral form

Please note that the Community Falls Service has no direct medical input and is not a Falls Clinic.

Exclusion Criteria:

  • Patients who have ongoing input from the Intermediate Care Team, Community Matron, District Nurse or Domiciliary Physiotherapist at the time of the referral, as these community healthcare professionals can also complete the falls assessment on relevant patients.
  • Patients who require physiotherapy for an injury as a result of a fall or for any other musculoskeletal problem, as these patients should continue to be referred to the adult domiciliary physiotherapy service or outpatient MSK physiotherapy service as appropriate.
  • Patients who have acute health or social care needs that require urgent assessment or ongoing monitoring/support – referral to the Intermediate Care Team may be more appropriate for patients requiring urgent assessment and daily or regular short-term support/multi-disciplinary input at home.
