Leeds Community Falls Service

The Leeds Community Falls Service is a citywide service providing specialist multi-disciplinary assessment and intervention consisting of physiotherapists, occupational therapists and therapy assistants.

The team will assess patients in their own homes (including residential homes) who have a history of falling, and require a multi-factorial falls risk assessment and therapy assessment/intervention in relation to the risk of falling.

From the assessment, the team will identify:

  • any falls risk factors
  • any further rehabilitation needs in relation to reducing the risks of falls – this may include providing exercise programmes (strengthening and balance exercises), assessment of mobility/appropriate walking aids/confidence with outdoor mobility, assessment of activities of daily living and provision of equipment where indicated, environmental home hazard assessments, anxiety management/advice in relation to fear of falling, facilitating participation in social activities and advice on coping strategies following a fall
  • the need for referral to other services and agencies
