The Community Falls Service is a citywide service consisting of Physiotherapists and Occupational Therapists. We provide specialist multi-factorial falls risk assessments and evidence-based therapy interventions to older people (60+ years old) in their own homes or in a community setting to reduce the risk of falling, and to maintain safety and independence.
The service also has links with the Falls Clinic (Leeds Teaching Hospitals NHS Trust) to refer relevant people who may require further specialist medical assessment regarding their falls.
The Community Falls Service sees falls patients who are registered with a Leeds GP and are deemed as complex and requiring the specialist knowledge, skills and expertise of the team. In general, patients who require a basic falls risk assessment and associated interventions will be directed to their local Neighbourhood Team for assessment.
If a patient is already under the care of a Neighbourhood Team, the Neighbourhood Team should be contacted in the first instance to complete a falls risk assessment and any relevant interventions. If not, then patients should contact their GP to discuss their needs further.
Please note that the Community Falls Service is not a rapid response service and referrals can only be made through a relevant healthcare professional in Leeds. For patients who require urgent assessment and/or short-term daily support at home with activities of daily living, referrals can be made by the patient’s GP to the Neighbourhood Team for a Rapid Response assessment.
