LCH is working with Leeds Older People’s Forum

A new programme designed to develop effective and innovative ways of supporting people to stay safely in their own homes is now live.

The new programme called Enhance is a partnership between Leeds Community Healthcare and Leeds Older People’s Forum and aims to support the safe and sustainable discharge of people from hospital and Neighbourhood Team care into a secure home environment. By linking the NHS Neighbourhood Teams with 14 third sector organisations the programme will avoid delays in hospital discharge, prevent readmissions, and increase the capacity in both sectors.

The third sector organisations involved in Enhance will provide a range of home and community-based services, which may include befriending, IT support, support with self-management, and undertaking joint visits with relevant health professionals, such as nurses or therapists.

Enhance will adopt throughout a ‘test and learn’ approach, where the development and testing of innovative solutions is encouraged. Five organisations - Armley Helping Hands, Age UK Leeds, Burmantofts Senior Action,  Care & Repair Leeds and Health for All - are already taking referrals, with others to follow soon. All 14 third sector partners will be taking referrals by the end of July.
