Leeds Community Healthcare NHS Trust runs services in a number of health centres and clinics across Leeds. These are used to provide a range of community-based health services to patients, for example podiatry, health visiting, continence services, contraceptive services and physiotherapy.
As part of our engagement on the locations of our services, people told us that there was more we could do to reduce some concerns about accessing services in our health centres. Next to each of the Health Centres listed below you will now see a downloadable document, where you can find transport information, details on where to park and access points for each Health Centre.
If you can’t find the information you need here, please call the Patient Advice and Liaison Service (PALS) on 0113 220 8585 Monday to Friday 8.30 am to 4.30 pm. Or you can email us on email@example.com. PALS give help, advice, information or guidance on all aspects of healthcare services in confidence.
