Joint Care Management

Joint Care Management is a partnership between NHS Leeds Community Healthcare and Leeds Adult Social Care (Leeds City Council). The teams provide a service to adults aged 65 years and over with complex health and social care needs and adults of all ages who have been identified as eligible for NHS Continuing Health Care funding.

The teams work in the acute hospitals and the community and work closely with Intermediate Care and the Community Intermediate Care Unit. (CICU).

Joint Care Management teams operate 8am to 8pm Monday to Friday and 9am to 5pm weekends and public holidays.

Referrals to the service can be made by any health or social care professional.
