Adult services

Adult services within Leeds Community Healthcare NHS Trust are split into three broad areas. They are:

• Long term conditions
• Out of hospital care
• Adult services

All these services are provided across the Leeds area to patients and their families.

We want to make sure that all our services:

• are delivered to the highest possible quality
• are innovative and effective
• aim to prevent illness as well as treat illness
• are provided in a way that centres on the patient and is responsive to each patient's individual and specific needs.

One of our key priorities is to ensure all services work together in an effective way. This will reduce duplication, improve patients' experiences and ensure best value.
